DATES Ride Forms

Ride Leaders,

Our liability insurance requires that we have riders sign waivers for each ride. This packet includes:

1.  6 Waiver Forms preprinted with DATES club member names, listed alphabetically by team captain's last name.
2.  1 Waiver form without any preprinted names for other riders.
3.  1 3-page document to be completed in case of accidents.  Follow the instruction on the forms.

 Print all the forms and take them to the ride.  Have all riders sign and date them and include a) cell phone # and b) license plate #.  After the ride, ask if anyone had an accident.  We will need to get the signed forms to the Rides Coordinator to be held in case someone comes forward with an injury or damage report we didn't know about.

 These forms are to be used for all DATES rides.  These forms will be provided at each DATES ride by the ride leaders or by the DATES Ride Coordinators.
